Copyright declaration forms

These forms have been developed to cover the use of third party material used in course materials. If you wish to include third party material in course readers or any other material being made available to students, please fill out the relevant form.

Monash University staff must ensure that their use of copyright material owned by third parties complies with Copyright legislation and licence requirements.

When you are requesting items to be made into course books or course materials by the Off Campus Learning Centre and the Audio Visual Production Unit MAPA, you must use the declaration forms below.

Unit readers and supplementary material
This includes journal articles, books, lyrics, tables, media releases and internet content.

Images
This includes photos, diagrams, illustrations, charts and cartoons.

Audiovisual items
This includes videos, DVDs and television broadcasts.

Music and Audio
This includes sound recordings, CDs, MP3s, and radio broadcasts.
