  1. Your name, address, phone number and email address so that we can contact you.
  2. As much detail as possible, including the name and nature of the material that you believe infringes copyright.
  3. The location of the material, with enough information so that we can find and access the material.
  4. Whether you are the owner of the copyright material. If not, in what capacity you are acting.
  5. Why you think the material is infringing copyright.
